A rebate card is attached to the rebate voucher your doctor will give you when you’re prescribed Evamist for the first time. Take the rebate voucher with you when you get your prescription filled. The pharmacist will give you a rebate card with an activated number called the cardholder id. If you do not have a rebate voucher or a rebate card, please call 1-877-567-7676 and request to have one sent to you. You will need a prescription for Evamist from your physician in order to take part in this offer.
